As we celebrate Mental Health Awareness Month in May, we want to hear from you. The ups and downs, what brings you peace, and everything in between.

While you don't have to answer every question, we would love to hear all of your thoughts to share with the community and reduce the stigma around mental health and the way we speak about it.

Submissions using this form may be anonymous; simply refrain from filling out your name and uploading a picture. Pseudonyms and a picture of your choosing to reflect your journey are encouraged if choosing to remain anonymous.

By submitting material (“Submission”), you agree that your Submission is an original work of authorship and you own all right, title and interest in the material submitted as of the date of the Submission. Submissions cannot defame or invade the publicity rights or privacy of any person, living or deceased, or otherwise infringe upon any person’s personal or property rights or any other third-party rights. If your Submission contains any material that is not owned by you, you are responsible for obtaining any and all releases and consents necessary to permit the use and exhibition of the Submission by the Colorado Rapids (“Rapids”), including name and likeness permissions from any person who appears in the Submission. If a person appearing in the Submission is under the age of majority in his or her state of residence, the parent or legal guardian of such person is required to provide permission. The Rapids reserve the right to disqualify any Submission deemed to be offensive, inappropriate or not in keeping with the Rapids’ image, in their sole discretion and to report any Submission that discloses a potential health or safety threat. By making a Submission, you further agree to assign all right, title, and interest, including copyright rights, in the Submission to Rapids and grant permission for Rapids to publish or publicize all or part of the Submission for advertising, promotional and trade or other purposes in any and all media now known or hereafter developed, worldwide in perpetuity, without notice or permission and without further compensation, except where prohibited by law. Submissions will be publicized at the discretion of the Rapids. Publication is not guaranteed. By making a Submission, you agree to release and hold harmless the Rapids from and against any and all claims, injuries, damages or losses that may occur, directly or indirectly, in whole or in part, from your participation. Any personally identifiable information collected during submittal of or participation in the Project will be collected by the Rapids and used as described in the Rapids’ privacy policy.
